How to Get a Listing in the getpalliativecare.org Palliative Care Provider Directory

There are two ways to get a listing:

Complete the brief Mapping Community Palliative Care survey (mapping.capc.org)

  1. Open to all palliative care programs across the continuum of care
  2. Requirements: Provide program information, identify the sites where your program provides care, and answer a few multiple choice questions about your program

Submit data to the National Palliative Care Registry™ (registry.capc.org)

  1. Open to all palliative care programs across the continuum of care
  2. Requirements: Provide program information, identify the sites where your program provides care, and submit data in an annual survey (at a minimum, provide the total number of initial consults your program provided per site)
  3. Programs participating in the Registry receive comparative reports that compare their program to peer programs on metrics like staffing
  4. These programs receive a premium listing in the Provider Directory
