How to Get a Listing in the GetPalliativeCare.org Palliative Care Provider Directory

To be listed in the directory, all palliative care programs must complete a Program Profile through the Palliative Care Quality Collaborative (palliativequality.org)

• Program profiles are open and free to all palliative care programs across the continuum of care
• Requirements: To be listed, a palliative care program must provide their contact information, identify the locations where their program provides care, and opt-in to being publicly listed in the Directory
• Update or confirm your program information annually to remain listed in the Directory
